This information is to assist parents who are registering their children at Sunset Park for the very first time. If you have any questions after reading all of the information, you may call the school at (305) 279-3222.

Things you will need to bring with you to register your child:

paw.gif Birth Certificate - Age verified by Birth Certificate or Alternate Document. A copy of the document used to verify age will be included in the Cumulative Record folder.

Completed HRS-H Form 3040, including proof of a tuberculin skin test, reading of the test, and appropriate follow-up.

Completed HRS Form 680, Part A, including proof of a tuberulin skin test, reading of the test, and appropriate follow-up, and the Hepatitis B series vaccination.

HEALTH REQUIREMENTS FOR SCHOOL ENTRANCE
Division of Attendance Services / March 1998

Florida law requires that your child present immunization documentation prior to admittance or attendance in a Florida school for the first time. This applies to all new students in pre-kindergarten through the 12th grade. You must present a Florida Certificate of Immunization, HRS Form 680-Part A, B, or C, when registering your child for school. NOTE: FOR THE 1998-99 SCHOOL YEAR, PARENTS MUST PROVIDE DOCUMENTATION OF TWO VALID MEASLES DOSES FOR CHILDREN ENROLLING/ATTENDING GRADES KINDERGARTEN THROUGH FIVE, AND FOR GRADES SEVEN AND EIGHT.

ADDITIONALLY, DOCUMENTATION OF THE HEPATITIS B VACCINE SERIES MUST BE PROVIDED FOR CHILDREN ENROLLING/ATTENDING GRADES KINDERGARTEN, SEVEN, AND EIGHT. A (Td) TETANUS-DIPHTHERIA BOOSTER, ADMINISTERED WITHIN THE PAST FIVE YEARS, IS ALSO REQUIRED FOR SEVENTH AND EIGHTH GRADE ATTENDANCE.

The "Florida Plan for School Health Services" requires that all students (PreK-12) submit documentation of a Student Health Examination performed within the 12 months prior to initial entry into a Florida school. A Student Health Examination (HRS-H Form 3040), including proof of tuberculin skin test, reading of test, and appropriate follow-up, should be completed and signed by a licensed practicing physician, and presented to the school at the time of registration.

Please consult your private physician or usual source of health care for the above requirement before registering your child for school. If you do not have a private physician or other source of health care, you may contact the school for the number of a Health Care Center for an appointment. Please have your child's record of immunizations with you at the time of your appointment.

Emergency Contact Cards must be completed and signed by the child's parent or guardian. If you have any address or phone number changes, please notify the school right away. This may save your child's life!
